Authority and Consent Form
Consent for participation in school activities and for travel.
Asthma Action Plan
All children who suffer from asthma must have an asthma plan lodged with the school.
Please ask for a copy of the below at reception or via the Parent Portal:
- Authorisation to administer paracetamol in an emergency
- Medical information and consent for camps
- Student Medication Request
This form must be completed for medication to be administered to your child during school hours.
